Job summary

Norfolk & Suffolk NHS Foundation Trust in Lowestoft is seeking a Band 6 practitioner to join the Waveney Adult Community Mental Health Team. The role focuses on care coordination under the Care Programme Approach for working-age adults, coordinating triage and referral assessments within a multidisciplinary team.

Applicants must hold NMC HCPC or Social Work England registrations as applicable and possess a full UK driving licence, with strong assessment and risk management skills for delivering

Qualifications

• Knowledge of mental health practice from previous experience.
• Community experience supporting working-age adults with mental health problems.
• Effective risk assessment and risk management experience.
• Experience delivering clinical interventions.
• Experience working in a team delivering person-centred treatment packages.

Responsibilities

• Plan and deliver direct patient care with minimal supervision, ensuring continuity of practice within a multidisciplinary and multi-agency setting.
• Manage a caseload and provide mental health care services to people with mental health needs.
• Act as a Care Coordinator under the Care Programme Approach.
• Take part in triage and assessment of referrals from GP surgeries and other services.
• Carry out mental health clustering of patients on admission, discharge and at review.
• Pass mental health clustering information to other Trust teams or services when patients transfer.
• Ensure the accuracy and quality of clustering activity within the team.
