NSFT, the Norfolk and Suffolk NHS Foundation Trust, is recruiting a Band 6 Mental Health Practitioner to work within North Norfolk GP practices. You will join a multidisciplinary primary care team, delivering non-urgent mental health assessments and brief interventions for adults 16+.

The role is linked to the ARRS funding and aims to provide care in familiar GP settings. The post supports a Place‑Based Care approach, embedded within North Norfolk 2 PCN, with opportunities to collaborate across

Qualifikationen

• Registered Mental Health or Learning Disability Nurse, Occupational Therapist or Social Worker with the relevant professional registration.
• Practice Assessor and Practice Supervisor training (NMC Registrants) or Practice Educator (AHP/SW Registrants).
• Ongoing Professional Development working towards degree level (if professional registration is following a diploma level course).

Aufgaben

• Provide assessments, brief interventions and short‑term support for people aged 16+.
• Support individuals with presentations including anxiety, depression, trauma‑related distress, bipolar disorder, psychosis and other mental health needs.
• Deliver psychoeducation, problem‑solving approaches, safety planning and self‑management strategies.
• Work collaboratively with GP practice staff and local services to ensure people are directed to the most appropriate support.
• Promote recovery, wellbeing and prevention of deterioration within local communities.
