Job summary

NSFT in Lowestoft is recruiting Band 6 Community Mental Health Practitioners to join the Waveney Adult Community Mental Health Team. You will be a qualified Nurse, Occupational Therapist or Social Worker with community experience under the Care Programme Approach.

The role focuses on triage and assessment within a multidisciplinary team, care coordination, and delivering direct patient care across working-age adults with mental health needs.

Qualifications

• Registered professional with UK registration in nursing, OT, or social work.
• Training as Practice Assessor/Supervisor or Educator in relevant register.
• Commitment to ongoing professional development toward degree level.
• Higher degree or postgraduate certificate in clinical/therapeutic practice.

Responsibilities

• Plan and deliver direct patient care with multidisciplinary support.
• Manage a caseload and provide mental health care to adults.
• Ensure accurate mental health clustering on admission, discharge and review.
• Pass clustering information to other Trust teams on patient transfer.
• Maintain accuracy and quality of clustering activity within the team.
