Pennine Care NHS Foundation Trust is recruiting for a role within the Neighbourhood Mental Health Team, an MDT operating in the transformed community mental health service for adults 18+ including older adults with serious mental illness. The post holder will ensure people are welcomed, listened to and directed to the right services, inside or outside the team.

You will work with service users to support access, act as keyworker in certain circumstances and help people engage in community

• Work within a multidisciplinary team to support adults 18+ including older adults with serious mental illness in the community.
• Collaborate with service users to route them to the most appropriate services inside or outside the Neighbourhood Mental Health Team.
• Act as service user keyworker in certain circumstances and support access to socially inclusive activities to maintain wellbeing and prevent relapse.
• Support goal-oriented care plans to empower service users to live as independently as possible and access housing, community groups, vocational/education opportunities and substance misuse services.
• Bridge primary and secondary care with VCSE partners to deliver high quality mental health and learning disability services across Greater Manchester.
