Pennine Care NHS Foundation Trust in Stockport is recruiting a Mental Health Support Worker for the Older Peoples Community Mental Health Memory Assessment Service. The role involves delivering timely mental health assessment, care planning and working with a multi‑agency team to prevent hospital admission.

You will work with Mental Health Nurses, provide direct care to service users in crisis related to physical health or social changes, and help model effective care with health and social care

Competenze

• Experience working in mental health services.
• Willingness to develop skills in mental health.
• Ability to work with multi‑agency teams.
