Job summary

Pennine Care NHS Foundation Trust is seeking a Mental Health Link Worker to join the Neighbourhood Mental Health Team. You will work with adults 18+, collaborate with the MDT, help service users access appropriate services, and act as keyworker in certain cases to support recovery and independence.

You will build care plans, signpost to community resources, and liaise with VCSE partners to promote wellbeing and prevent relapse across Greater Manchester's five boroughs, including Stockport.
