Northumbria Healthcare Trust is seeking a Community Matron to join our Community Nursing Teams across North Northumberland. You will identify vulnerable patients at risk and those frequently admitted to hospital, delivering proactive case management in partnership with Specialist Nurses, Medical colleagues, Palliative Care, Social Care and AHPs to reduce admissions and improve quality of life.
