Job summary

Hywel Dda University Health Board is seeking a Band 5 Occupational Therapist to join the Community Learning Disability Team. The role involves assessing and treating adults with learning disabilities, managing a caseload and working with people, families and partner services to support meaningful outcomes across hospital, community and home settings.

Requirements include a degree in Occupational Therapy and current HCPC registration (or pending).

Qualifications

• Current HCPC registration or confirmation pending.
• Experience in therapy or a relevant field, with caseload management.
• Knowledge of health and social care legislation and audit methods.
• Ability to travel between sites and work across hospital, community and home settings.
• Welsh language level 1 is a plus.

Responsibilities

• Assess and treat adults with learning disabilities.
• Manage a varied caseload and apply clinical reasoning to improve function.
• Deliver evidence-based interventions across hospital, community and home settings.
• Collaborate with patients, families, carers and partner services on personalised plans.
• Support hospital discharges and independence through equipment and adaptations.
• Contribute to service development and professional learning within the team.
