Job summary

Pennine Care NHS Foundation Trust in Hyde is seeking a Community Learning Disability Nurse at NHS AfC Band 6 to deliver specialist health assessments and coordinated care for adults with learning disabilities.

The role involves working within a multi-disciplinary team across community and home settings, developing personalised care plans, and providing support to carers and families while upholding public health goals.

Qualifications

• Registered nurse with Learning Disabilities (RNLD) registration.
• Post-registration training or willingness to undertake training to postgraduate diploma level.
• Experience in community learning disability services preferred.

Responsibilities

• Provide specialist health and behaviour assessments.
• Deliver comprehensive assessments for individuals with learning disabilities and complex needs.
• Coordinate clinical care within a defined caseload with supervision as appropriate.
• Offer safeguarding and risk management as part of the care plan.
