Job summary

Qualifications

Responsibilities

• Manage a busy caseload within the community setting.
• Work autonomously and as part of a multidisciplinary team.
• Provide assessment and treatment for physical and mental health needs.
• Collaborate with social care and external providers to support individuals with intellectual disabilities.
• Contribute to care and risk management planning and documentation.
