An exciting permanent, part-time opportunity has arisen for a motivatedand compassionate Band 5 Community Staff Nurse to join our Out of HoursCommunity Nursing Service in Halton covering the evening service between 17:0023:00 3 x shifts a week including Saturdays and Sundays.This role is ideal for a registered nurse who is enthusiastic about deliveringhigh-quality, person-centred care to patients in their own home and supportingpeople to remain safe and independent within the community.You will work as part of a supportive and integrated team, caring for houseboundpatients with a range of nursing needs including wound care, palliative care,catheter care and long-term condition management.We are looking for someone who communicates effectively, works well bothindependently and as part of a team and is committed to continuous professionaldevelopment. The post holder plays a vitalrole in supporting the District Nursing team through the management of anassigned caseload.Key responsibilities encompass the assessment, planning, and evaluation ofpatient care needs.
The incumbent works closely withthe District Nurse to facilitate effective communication regarding patientcare, maintain up-to-date care plans, and ensure seamless continuity ofservices, especially during reviews of complex care arrangements. Additionalduties include contributing to mentoring, educational, and assessmentactivities. Collaboration with healthcare professionals and external agenciesis integral to providing efficient and high-quality service delivery.

The postholder actively supports the District Nursing team by undertakingresponsibility for a delegated caseload.
Undertakesthe assessment, planning and evaluation of patients care needs.
Liaiseswith the District Nurse to ensure information sharing in relation to patientcare / changes to care plan and the continuity of care and reviewing of complexcare packages.
Mentoring,Teaching and Assessing.
Liaiseswith all health care professionals and other agencies to ensure effectiveservice delivery.
To promotethe health and wellbeing within the family unit and community as a whole.
Performcomprehensive assessment of patient nursing needs as the named nurse.
Plan,assess and evaluate programmes of care according to changing health needs underthe direction of the District Nurse
To supportthe team leader in the co-ordination of complex and continuing care packages.
Maintainaccurate records in accordance with NMC.
Attend amulti-disciplinary case conference as the named nurse if appropriate.
Tocommunicate and collaborate with other members of the PHCT, voluntary andstatutory organisations to support individual patient care.
Toestablish and maintain effective communication with patients, relatives andcarers in complex and potentially challenging situations.
Tocompetently perform clinical skills related to the post.
To beprofessionally responsible for a delegated caseload under the supervision ofthe District Nurse.
To assesshealth needs at individual, practice and community level.
Toundertake chronic disease management reviews using appropriate template.
To work without directsupervision.
