Job summary

Waythrough is seeking a Support Coordinator to provide high-quality, person-centred floating support and housing management to adults with multiple disadvantages across the Bradford District. You will empower people to sustain accommodation and improve wellbeing in home and community settings.

The role requires working evenings/weekends as needed, with a focus on safeguarding and multi-agency collaboration to reduce homelessness and promote independence.

Qualifications

• NVQ Level 3 in Health & Social Care (or willingness to work towards)
• Knowledge of welfare benefits system and relevant legislation
• Knowledge of housing options and relevant legislation
• Knowledge of safeguarding practice and relevant legislation
• Experience of multi-agency working with external partners
• Experience of supporting vulnerable people and risk management
• Experience of managing a caseload and promoting independence
• Ability to communicate well and maintain accurate records
• Ability to drive and own a vehicle used for work
• Proficient in Microsoft Outlook Office programmes

Responsibilities

• Support clients to live independently in their own accommodation and progress towards sustained housing.
• Deliver person-centred, strengths-based and trauma-informed support, enabling positive outcomes and wellbeing.
• Provide housing-related support to help clients establish tenancies and access services.
• Collaborate with partner agencies to deliver coordinated support and manage risk.
• Attend regular team meetings and support colleagues across sites.
• Manage caseloads and complete needs and risk assessments.
• Develop comprehensive support/risk plans to meet individual needs.
• Undertake outreach to referral agencies to promote the service and handle referrals.
