Job summary

Waythrough is seeking a dedicated housing support professional to assist adults with multiple disadvantage in Bradford District. You will work in clients' homes and in the community to empower independence, wellbeing, and sustained housing.

The role requires delivering person-centred, strengths-based support and coordinating with partner agencies to ensure timely, effective service delivery. The position involves managing caseloads, completing needs and risk assessments, and working flexible

Qualifications

• NVQ Level 3 in Health & Social Care or equivalent.
• Knowledge of welfare benefits and relevant legislation.
• Knowledge of housing options and relevant legislation.
• Knowledge of safeguarding practice and relevant legislation.
• Experience of multi-agency working with external partners.
• Experience of supporting vulnerable people, including support planning and risk management.

Responsibilities

• Support clients to live independently in their own accommodation and progress towards sustained independent housing.
• Deliver person-centred, strengths-based and trauma-informed support to enable positive outcomes.
• Provide housing-related support to establish tenancies and prevent homelessness within the community.
• Collaborate with partner agencies to deliver coordinated support and manage risk.
• Manage a caseload and complete high-quality needs and risk assessments.
