Job summary

Reed in Partnership is recruiting a Neighbourhood Work & Health Coach to provide personalised, person-centred support for individuals facing health and employment barriers. You will conduct biopsychosocial assessments, develop Work and Health Action Plans, and work with healthcare professionals, employers and community services to enable sustainable employment and improved wellbeing.

Qualifications

• Experience delivering personalised advice, guidance, coaching or support services to adults facing barriers to employment, health or wellbeing.
• Experience managing a caseload and supporting individuals to achieve agreed outcomes.
• Experience working in partnership with multiple organisations and stakeholders.
• Experience working to performance standards and quality requirements.
• Strong understanding of the impact that physical health, mental health and disability can have on employment.
• Minimum of 2 A-Levels or an equivalent Level 3 Diploma.
• GCSE English Language at a minimum of Grade B (or equivalent Level 2 qualification). OR Level 2 Literacy assessment at interview.

Responsibilities

• Conduct comprehensive biopsychosocial assessments to understand health, wellbeing and employment.
• Build trusted relationships with participants through person-centred coaching.
• Use motivational interviewing and behaviour change techniques to identify strengths and barriers.
• Develop personalised Work and Health Action Plans including employment and wellbeing goals.
• Identify barriers affecting employment and provide ongoing coaching and support.
• Help participants improve confidence, wellbeing and work readiness.
• Support return-to-work or employment transitions with structured plans.
• Coordinate interventions and referrals to address barriers and ensure timely support.
• Provide employment coaching including CV development and interview preparation.
• Coordinate with clinicians and health professionals to deliver integrated support plans.
• Elevate safeguarding, welfare and risk concerns per policy.
