Leicestershire Partnership NHS Trust is seeking enthusiastic individuals to join its Community Nursing Teams. You will provide care to patients in their own homes, with a requirement to drive and have a car for work.

You will work under a District Nurse and alongside registered community nurses to deliver a wide range of care activities. Main duties include clinical care such as wound/leg ulcer management, medication administration and catheter care, plus personal care, and collaboration with

• Level 2 qualification in Maths and English or equivalent.
• Level 3 qualification in Care or equivalent relevant experience.

Responsabilidades

• Providing clinical care delegated to you by a Registered Nurse, this includes, but is not limited to: management of wounds and leg ulcers, administration of medicines and catheter care.
• Providing personal care for patients.
• Working with the wider multidisciplinary team.
• Supporting patients to remain at home and avoid hospital admission.
• Promoting self-care and independence.
• To recognise and report changes in the patients condition, and any issues in respect of their health and management of care to the registered practitioner.

Educación

Level 2 qualification in Maths and English or equivalent

Level 3 qualification in Care or equivalent relevant experience
