Zusammenfassung

HCRG Care Group is seeking a Community Healthcare Assistant to join our Malmesbury-based team. The role supports patients in their homes and local community settings, delivering person-centred care under clinical guidelines.

The successful candidate will work 22.5 hours per week, travel across the locality, and contribute to rehabilitation and independence. A pro-rata salary range is provided with pension and benefits.

Qualifikationen

Aufgaben

• Deliver high-quality care and support to patients in community settings.
• Assist with personal care and activities of daily living where required.

• Monitor and report changes in patients' health and wellbeing.
• Work closely with nurses, therapists, GPs, and other healthcare professionals.
• Maintain accurate patient records and documentation.
• Follow care plans, policies, procedures, and clinical protocols.
• Promote dignity, respect, and person-centred care at all times.
• Support patients and families to achieve positive health outcomes.
