Job summary

HCRG Care Group is seeking a Community Health Assistant to join the 0-19 Children and Families public health team in Walton-on-Thames. You will assist in delivering care and support across home, community, schools and clinic settings, delivering a high standard of family-centred care.

The role supports universal and specialist services to improve health outcomes for children and families, under supervision. You will carry out assigned tasks and adapt care packages as progress and outcomes show.

Qualifications

• GCSEs in Maths and English (min 4)
• NVQ4 / CACHE 3 Diploma or BTEC National Diploma in Children’s Care, Learning and Development or transferable skills with children
• Diploma or Degree in Health and Social Care required
• Previous team-working experience
• Confidentiality and information governance knowledge
• Good organisation and time management
• Ability to build positive relationships with internal and external partners

Responsibilities

• Deliver care packages to children, young people and families in homes, schools, clinics and community settings as directed.
• Support assessments, monitor progress and contribute to care reviews using approved tools under supervision.
• Implement individual and group interventions and co-facilitate training, workshops and community programmes.
• Identify safeguarding concerns according to Surrey safeguarding procedures and act accordingly.
• Build strong relationships with families, schools and multi-disciplinary teams.
• Maintain professional boundaries and communicate sensitively with families.
• Complete administrative tasks and support equipment maintenance and procurement.
