Job summary

Buckinghamshire Healthcare NHS Trust in Wycombe is seeking a Community Practitioner with a broad range of clinical skills to join the Urgent Community Response Frailty Hospital at Home team. You will provide rapid, hospital-level assessments and treatments in the patient’s home, coordinate care with physiotherapists, occupational therapists and other colleagues, and help reduce unnecessary hospital visits.

Qualifications

• Registered Healthcare Professional with HCPC and Degree in Physiotherapy.
• Experience of NHS community/in-patient setting.
• Minimum 2 years post-registration NHS experience.
• Training in Long Term Conditions, Respiratory Disease, Cardiac Disease, Trauma, Orthopaedics & End of Life.

Responsibilities

• Respond to 0-2 hour and 2-24 hour community response referrals, plus planned follow-ups, delivering hospital-level treatments at the patient’s home.
• Assess physical and psycho-social needs and provide evidence-based interventions.
• Improve health outcomes using professional skills and knowledge.

Degree in Physiotherapy
