Buckinghamshire Healthcare NHS Trust is recruiting a Community Practitioner Physiotherapist to join the Wycombe team. You will provide expert physiotherapy interventions and rehabilitation in patients’ homes, supporting independence and early discharge from hospital.

You will work within a multi-disciplinary team, triaging, assessing and planning care with colleagues across nursing, occupational therapy, paramedics and pharmacists.

Formación

• Registered physiotherapist with HCPC registration and active practice in community settings.
• Experience delivering specialist physiotherapy assessments and rehabilitation in patients at home.
• Ability to work as part of a multi-disciplinary team in rapid response and intermediate care.

Responsabilidades

• Provide expert physiotherapy interventions and rehabilitation in community settings.
• Carry a community physiotherapy caseload and assess patients in their homes.
• Plan care programmes to promote independence and prevent hospital admission.
• Allocate workloads within the RRIC team and coordinate with partners across health and social care.
