Job summary

Buckinghamshire Healthcare NHS Trust is seeking a qualified Physiotherapist to join the Rapid Response, Intermediate Care and Community Physiotherapy team in Aylesbury. The post is Band 6, part‑time (22.5 hours per week) with flexible working and a 12‑month secondment/fixed term arrangement.

The role involves triage, specialist assessments, case management, and delivering care in the community to prevent admission and support early discharge.

Qualifications

• Registered healthcare professional with HCPC or RCN registration.
• Degree or diploma in Physiotherapy (or relevant field).
• Experience in NHS community settings and multidisciplinary work.

Responsibilities

• Provide triage, specialist assessment, and evaluation of patient needs.
• Plan programmes of care to promote health gain and independence.
• Allocate workloads within the Rapid Response & Intermediate Care team.
• Collaborate with a range of clinicians to ensure timely, appropriate care.
