Zusammenfassung

Codi Group is seeking a full-time Community Engagement Project Worker for our Carmarthenshire Prevention Hub, covering Llanelli, Ammanford and Gwendreath. You’ll support customers with information, build local networks and deliver preventative, solution-based community support to improve health, wellbeing and independence.

As part of the lead team, you’ll be the single point of contact for access to information and services.

Qualifikationen

• Empathy and people skills to support diverse customers.
• Ability to work independently and as part of a team.
• Tact and resilience in challenging situations.
• Strong relationship-building with customers and partners.
• Ability to plan, deliver, monitor and review a holistic support package.
• Ability to assess risk and propose mitigation strategies.
• Hold a driving licence and have access to a car.

Aufgaben

• Deliver preventative hubs in Llanelli, Ammanford and Gwendreath.
• Act as single point of contact to provide information and access to services.
• Collaborate with partner organisations to deliver services and campaigns.
• Receive referrals, have wellbeing conversations and develop community support networks.
