Job summary

Codi Group is seeking a full-time Community Engagement Project Worker for the Carmarthenshire Prevention Hub, covering Llanelli, Ammanford and Gwendreath. You will be central to delivering preventative and solution-based support to help customers improve health, well-being and independence through social integration and strong community relationships.

As part of the lead team, you will be the single point of contact for information, opportunities and support, working with partner organisations

Responsibilities

• Deliver the single point of contact from which customers can access information, opportunities and support.
• Work with partner organisations to deliver preventative hubs in Llanelli, Ammanford and Gwendreath.
• Receive referrals, provide information, have wellbeing conversations, deliver community engagement campaigns and develop community support networks.
• Assess risk and propose risk management strategies.
• Hold a driving licence and have access to a car.
