Job summary

The Methodist Church in Britain is seeking a Junior Sister/Junior Charge Nurse to work within the Braintree ICT primary health care team. You will plan, allocate and review patient care, manage caseloads and coordinate complex discharges with palliative care when needed.

Candidates must hold current NMC registration, possess a district nursing course, and have at least three years post-registration experience. Travel across bases may be required.

Qualifications

• Current NMC registration in district nursing or equivalent.
• District nursing course completed or actively pursued.
• Evidence of ongoing continuing professional development.
• Three years post-registration experience in community/nursing care.

Responsibilities

• Manage caseloads, including allocating patients and reviewing visit frequency.
• Formulate and review care plans and evaluate care across visits.
• Lead on assessment, implementation and evaluation of care.
• Take charge of the team in the Community Matron's absence when designated.
• Coordinate care and complex discharges home, including palliative care.
• Educate patients and carers to promote health; liaise with multidisciplinary teams.
