Job summary

Buckinghamshire Healthcare NHS Trust is seeking a part-time District Nurse / Community Case Manager for the Amersham Adult Community Healthcare Team. The role covers 30 hours per week with weekend and late shifts, delivering in-home nursing care and complex case management across Buckinghamshire.

You must hold UK nursing registration and have community experience; driving is essential to reach patients at home. Induction and competency development are provided.

Qualifications

• Registered Allied Healthcare Professional with UK registration.
• Experience in community health settings.
• Willingness to undertake the District Nurse Training.

Responsibilities

• Provide nursing interventions in patients' home environment.
• Manage caseloads for high-risk patients to prevent deterioration.
• Coordinate with MDT and arrange referrals for community care.
• Deliver care delivery based on best available evidence.
• Support medication management and complex care needs.
