Wrightington, Wigan and Leigh Teaching Hospitals NHS Foundation Trust is seeking a Community Staff Nurse (District Nurse) to join the Wigan Central District Nursing team. The post involves visiting patients at home across the Trust and requires a registered nurse with valid NMC registration and a commitment to ongoing professional development.

The role is 37.5 hours per week with a 08:30–17:00 pattern, including weekend work as required.

Mansioni

• Deliver evidence-based nursing care as delegated by District Nurse Coordinator/Team Leader.
• Perform skilled clinical nursing procedures and provide clinical advice.
• Support patients with complex health care needs and coordinate care plans.
• Assess, plan, implement, and evaluate care using district nurse criteria.
• Communicate changes in patient condition to the District Nurse Coordinator/Team Leader.
• Ensure care is delivered via appropriate referrals to other agencies.
