• Enthusiastic and motivated nurse with an interest in diabetes.
• Ability to deliver structured education to those newly diagnosed with type 2 diabetes.
• Ability to educate healthcare professionals.

Responsibilities

• Provide specialist nursing service to adults living with diabetes as part of a small nurse-led team.
• Deliver care, education and support to people living with diabetes in primary care.
• Help patients reduce risks of acute and long-term complications of diabetes.
• Deliver education to healthcare professionals.
• Participate in planning and delivery of diabetes care as part of a wider multi-professional team.
• Participate in service improvement and transformation initiatives to improve diabetes outcomes.
