We are seeking an enthusiastic and motivated Community Specialist Nurse (Diabetes) to join the Community Diabetes Service. The successful candidate will work as part of a multidisciplinary team to deliver high-quality, evidence-based diabetes care, supporting people with diabetes to effectively manage their condition and achieve improved health outcomes.
The post holder will provide specialist assessment, treatment, education, and ongoing support to patients within community settings, working closely with service users, carers, primary care colleagues, and partner organisations. They will contribute to the planning, implementation, and evaluation of individualised care plans, ensuring care is delivered in line with current clinical evidence, national guidance, and local service priorities.
The role offers the opportunity to develop specialist expertise in diabetes management while supporting service development, delivering education and training to healthcare professionals, and participating in quality improvement, audit, and research activities.
