Job summary

HCRG Care Group seeks an experienced Diabetes Specialist Nurse to deliver expert assessment, education and support to patients with diabetes, within community and primary care settings. You will work in a multidisciplinary team with autonomy in patient management and contribute to service improvement and clinical governance.

Required are post-registration diabetes experience, NHS policy knowledge, and the ability to educate patients and healthcare professionals.

Qualifications

• Post-registration development in diabetes nursing.
• Significant post-registration experience in diabetes.
• Experience supporting diabetes patients in acute, community or primary care settings.
• Knowledge of NICE guidance and NHS policy.

Responsibilities

• Deliver specialist diabetes nursing care across community and primary care.
• Undertake complex clinical assessments considering physical, psychological, social and cultural factors.
• Initiate and review diabetes treatment plans per local and national guidance.
• Work autonomously within a multidisciplinary team.

Job Description

This is an exciting opportunity for an experienced nurse with a passion for diabetes care to join our community-based specialist team. The role focuses on delivering expert assessment, treatment, education, and support to patients with diabetes, their families, and carers. The successful candidate will work within a multidisciplinary team while exercising a high degree of professional autonomy in the management of patients with diabetes. They will contribute to the delivery of safe, effective, and patient-centred care, support service improvement activities, and work collaboratively with colleagues across Wiltshire to ensure consistently high standards of practice in line with local and national guidance.

Responsibilities

• Deliver specialist diabetes nursing care to patients across community and primary care settings.
• Undertake complex clinical assessments, identifying physical, psychological, social, and cultural factors that may impact care.
• Initiate, review, and evaluate diabetes treatment plans in accordance with local and national guidance.
• Work autonomously while contributing effectively within a multidisciplinary team environment.
• Provide specialist advice, education, and support to patients, carers, and healthcare colleagues.
• Contribute to clinical governance, service development, audit, and quality improvement activities.
• Support the development of standards, protocols, and safe clinical practice across the service.
• Maintain high-quality clinical documentation and participate in team and professional network meetings.
• First Level Registered Nurse.

Requirements

• Evidence of post-registration development in diabetes nursing.
• Significant post-registration experience within the speciality of diabetes.
• Experience supporting patients with diabetes in acute, community, or primary care settings.

Knowledge and Skills

• Current knowledge of diabetes care, NICE guidance, and evidence-based practice.
• Understanding of relevant NHS legislation and policy.
• Excellent communication and presentation skills.
• Ability to interpret and analyse complex clinical information.
• Effective workload management and prioritisation skills.
• Ability to work independently and use professional judgement.
• Competent IT skills., Degree or Diploma in a health-related subject.
• Independent Non-Medical Prescriber qualification.
• Experience undertaking clinical audits.
• Experience working within multidisciplinary teams.
• Experience delivering education to healthcare professionals and patients.
• Experience developing protocols and supporting service improvement.
