Job summary

HCRG Care Group seeks an experienced Diabetes Specialist Nurse to join our community-based specialist team in Salisbury. You will deliver expert assessment, treatment, education, and support to patients with diabetes, their families and carers.

The role offers autonomous practice within a multidisciplinary team, contributing to safe, patient-centred care and service improvement across Wiltshire in line with national guidance.

Qualifications

  • Registered Nurse with post-registration diabetes experience.
  • Experience delivering diabetes care in acute/community settings.
  • Knowledge of NICE guidance and evidence-based practice.
  • Ability to work in multidisciplinary teams and contribute to service development.
  • Excellent communication and documentation skills.

Responsibilities

  • Deliver specialist diabetes nursing care to patients in community and primary care.
  • Undertake complex clinical assessments considering physical, psychological, social, and cultural factors.
  • Initiate and evaluate diabetes treatment plans per local and national guidance.
  • Work autonomously within a multidisciplinary team environment.
  • Provide education and support to patients, carers, and colleagues.
  • Contribute to governance, audit, and quality improvement activities.
  • Support development of standards, protocols, and safe practice across the service.

Supporting High-Quality Diabetes Care Across the Community

This is an exciting opportunity for an experienced nurse with a passion for diabetes care to join our community-based specialist team. The role focuses on delivering expert assessment, treatment, education, and support to patients with diabetes, their families, and carers.

The successful candidate will work within a multidisciplinary team while exercising a high degree of professional autonomy in the management of patients with diabetes. They will contribute to the delivery of safe, effective, and patient-centred care, support service improvement activities, and work collaboratively with colleagues across Wiltshire to ensure consistently high standards of practice in line with local and national guidance.

Key Responsibilities
  • Deliver specialist diabetes nursing care to patients across community and primary care settings.
  • Undertake complex clinical assessments, identifying physical, psychological, social, and cultural factors that may impact care.
  • Initiate, review, and evaluate diabetes treatment plans in accordance with local and national guidance.
  • Work autonomously while contributing effectively within a multidisciplinary team environment.
  • Provide specialist advice, education, and support to patients, carers, and healthcare colleagues.
  • Contribute to clinical governance, service development, audit, and quality improvement activities.
  • Support the development of standards, protocols, and safe clinical practice across the service.
  • Maintain high-quality clinical documentation and participate in team and professional network meetings.
Qualifications and Experience
  • First Level Registered Nurse.
  • Evidence of post-registration development in diabetes nursing.
  • Significant post-registration experience within the speciality of diabetes.
  • Experience supporting patients with diabetes in acute, community, or primary care settings.
Knowledge and Skills
  • Current knowledge of diabetes care, NICE guidance, and evidence-based practice.
  • Understanding of relevant NHS legislation and policy.
  • Excellent communication and presentation skills.
  • Ability to interpret and analyse complex clinical information.
  • Effective workload management and prioritisation skills.
  • Ability to work independently and use professional judgement.
  • Degree or Diploma in a health-related subject.
  • Experience undertaking clinical audits.
  • Experience working within multidisciplinary teams.
  • Experience delivering education to healthcare professionals and patients.
  • Experience developing protocols and supporting service improvement.
