Job summary

Central London Community Healthcare NHS Trust is seeking an enthusiastic Community Specialist Nurse (Diabetes) to join the Community Diabetes Service. You will work within a multidisciplinary team to deliver high-quality diabetes care and support people to manage their condition.

You will provide specialist assessment, education, and ongoing support to patients in community settings, helping them to achieve better health outcomes and self-management.

Responsibilities

• Provide specialist assessment, treatment, and management for adults with diabetes in community settings.
• Support and empower patients to self-manage their diabetes and improve health outcomes.
• Deliver expert advice on diabetes treatments, including oral medications and insulin therapy.
• Independently assess, plan, implement, and evaluate personalised care plans.
• Work collaboratively with multidisciplinary teams and primary care partners to coordinate care.
• Manage referrals and provide timely specialist support for patients with complex diabetes needs.
• Educate and support patients, carers, families, and healthcare professionals.
• Contribute to service improvement, clinical audit, research, and quality initiatives.
• Promote and implement evidence-based practice, clinical standards, and guidelines.
• Supervise, mentor, and support colleagues, students, and wider healthcare staff.
• Maintain accurate and comprehensive clinical records.
• Uphold professional accountability in line with the NMC Code and engage in continuous professional development.
