Job summary

5 Star Recruitment is seeking a Community Care Officer Level 2 - Adults to work under the Lead Practitioner to assess needs, develop tailored care packages and support service users to maximise independence.

The role involves coordinating with social workers, therapists and partner agencies, managing budgets and monitoring progress, while ensuring adherence to policies and confidentiality.

Qualifications

• GCSEs including English and Maths
• Experience of working with families and networks to achieve identified outcomes.
• Understanding of risk and experience in developing solutions to manage identified risks.
• Ability to manage conflicting priorities under guidance.
• Understanding of confidentiality and rights of service users and carers.
• Knowledge of legislation relevant to health and social care.

Responsibilities

• Hold and manage a caseload of service users with a higher level of need.
• Provide support to Qualified Social Workers / therapists as required.
• Develop care packages in consultation with carers and families.
• Work in partnership with other agencies to maximise outcomes for service users.
• Assess needs including non-social care requirements to design and record care packages.
• Work as part of a multi-disciplinary team with other agencies.
