Qualifications

• Ability to assist and support citizens to exercise choice and control.
• Encourage, motivate and support citizens to maintain or regain independence.
• Develop and coordinate leisure and social activities to promote self-esteem and purpose.
• Write and maintain up-to-date individual person-centred care plans.

Responsabilités

• Care Workers at Level 2 implement Care Plans and monitor welfare.
• Collaborate with other professionals and citizens to ensure continuity of care.
• Maintain running records and accurate documentation.
• Administer medication as required.
