Job summary

5 Star Recruitment is seeking a Community Care Officer Level 2 - Adults to work under the Lead Practitioner to assess needs, develop tailored care packages and support service users to maximise independence.

The role involves coordinating with social workers, therapists and partner agencies, managing budgets and monitoring progress, while ensuring adherence to policies and confidentiality.

Qualifications

About the job Community Care Officer Level 2 - Adults

PURPOSE OF JOB:

To work as directed by the Lead Practitioner with people who may require care and support to

identify needs, strengths and outcomes and provide a range of services to support people to

manage their needs and maximise their independence.

MAIN DUTIES:

• To work within the policies, practices of the Adult Care and Community Wellbeing
  Directorate to prescribed standards and to participate in learning and development asrequired by the Lead Practitioner and Continuous Professional Development.
• Hold and manage a caseload of service users with a higher level of need. This will includethe identification of service users and the provision of information and advice. Including apotential for a complex response assessment of user and career need, commissioningfollow on care and monitoring and review, amending as required. Operating within budgetframework set by the Directorate and managed by the Lead Practitioner.
• Provide support to Qualified Social Workers / therapists as required in line with the serviceroles and responsibilities.
• Contribute, co-operate and work in partnership with other agencies to ensure serviceusers are safe and protected and to maximize the outcomes for service users.
• To provide a range of equipment, arrange minor adaptation work and to teach alternativetechniques to overcome the effects of disability on independent living where appropriate.
• Understand the value and usefulness of performance information and uses data to monitorservice user progress and the teams overall performance.
• Represent the Directorate at multi-agency meetings. Share written / verbal information toboth internal and external parties in line with Directorate policies and procedures and legalrequirements.
• To undertake assessment of needs including any non-social care requirements such ashealth care or housing to enable planning, design and recording care packages in
  agreement with service users, carers and other agencies within available resources andbudgetary constraints.
• Develop care packages to include the design of individually tailored care packages forservice users in consultation with identified carers/ families.
• To work as a team member within the multi-disciplinary context and to develop
  professional working relationships with other agencies including Health Care Services,Housing Authorities and Voluntary and Independent Sector Services.

PERSON SPECIFICATION

• Good standard of general education with a minimum of 4 GCSE passes at Grade C or above or equivalent including English and Math's
