Job summary

Bharani Medical Centre in Slough seeks an experienced Clinical Pharmacist to join our NHS GP team. You will work with EMIS Web, Ardens, and primary care workflows to support medicines safety, optimisation and long-term condition management.

The role offers flexible working, including remote or hybrid arrangements, with potential for self-employed initial engagement and progression to salaried employment.

Qualifications

• Qualified clinical pharmacist with NHS general practice experience.
• Familiarity with EMIS Web, Ardens and primary care clinical workflows.
• Experience with medicines reconciliation and QOF.
• Ability to work with high degree of professional independence.

Responsibilities

• Review hospital discharge summaries and clinical correspondence.
• Undertake medicines reconciliation and update EMIS Web records.
• Review QOF registers and perform structured medication reviews for diabetes, hypertension, COPD, and asthma.
• Identify patients requiring monitoring and optimisation; arrange follow-up.
• Conduct prescribing safety searches for high-risk medicines.
• Perform medicines optimisation reviews including gabapentinoids and CKD.
• Use EMIS Web and Ardens to identify relevant patient cohorts.
• Conduct remote or face-to-face medication reviews with documented actions.
