Job summary

Roby Medical Centre in the UK is recruiting Clinical Pharmacists to support medicines optimisation across up to two PCN practices within the PCN, funded by ARRS. The role includes structured medication reviews, prescribing within NICE guidelines, and contributing to patient safety and chronic disease management.

The post sits within a governance framework with named supervision, offering flexible full-/part-time arrangements and hybrid working across the PCN practices.

Qualifications

• Registered Pharmacist with the General Pharmaceutical Council (GPhC).
• Independent Prescriber qualification (or completion by start date).
• CPPE Primary Care Pharmacy Education Pathway (PCPEP), or at least 80% completed at application.

Responsibilities

• Undertake Structured Medication Reviews (SMRs) for polypharmacy and high-risk medicines.
• Provide medicines optimisation advice to GPs and the MDT.
• Prescribe and deprescribe within scope, following NICE guidance and local formularies.
• Support long-term condition management (CVD, diabetes, respiratory disease, frailty).
• Monitor and manage high-risk medicines (anticoagulants, DMARDs, controlled drugs).
• Contribute to antimicrobial stewardship and prescribing quality improvement.
• Participate in audits and medicines safety activities.
