Job summary

Pembroke Medical Group in Paignton is seeking a Clinical Pharmacist to deliver high-quality patient care, medicines optimisation, and support for long-term condition management within a busy GP practice.

You will work closely with GPs and the wider clinical team, conduct structured medication reviews, improve prescribing safety, and contribute to quality improvement and medicines optimisation initiatives.

Qualifications

• Undegraduate degree in pharmacy (MPharm) or equivalent.
• Independent prescriber qualification required.
• Postgraduate Certificate in Pharmacy Practice desirable.

Responsibilities

• Minimum of 2 years experience post-qualification.
• In depth therapeutic and clinical knowledge and understanding of evidence-based medicine.
• Good knowledge of service developments in medicines, prescribing and NHS policy.
• Ability to plan, prioritise, manage and review medicines optimisation issues in core areas for long-term conditions.
• Excellent communication, interpersonal and negotiation skills.
• Experience in audit, research and analysis of highly complex information related to improving clinical quality.
