Job summary

Gtd Healthcare is recruiting a Clinical Practice Based Prescribing Pharmacist to Hattersley Group Practice in Hyde. You will work within a busy team, supporting medicines optimisation and delivering patient-centred care across primary and urgent care settings.

The role requires experience in primary care prescribing, operational leadership, and collaboration with GP and pharmacy colleagues to improve outcomes.

Qualifications

• Experience working in primary care/general practice as a clinical pharmacist.
• Ability to communicate complex information to patients and colleagues.
• Ability to review medicines optimisation and prescribing decisions.

Responsibilities

• Provide prescription support and manage medication queries and repeat prescriptions.
• Deliver medicines management advice via telephone, video, or face-to-face.
• Lead on prescribing-related governance and collaboration with ICB/PCN pharmacists.
• Undertake structured medication reviews for patients with polypharmacy.
• Develop and implement innovative services to meet service needs.
