Job summary

gtd healthcare is recruiting a Clinical Practice Based Prescribing Pharmacist to join Hattersley Group Practice. You will support GP medicines optimisation, provide clinical medicines advice, and deliver safe, expert care within a proactive clinical pharmacy team.

The role emphasizes collaboration across primary care, community, and hospital pharmacy, with opportunities to innovate and develop new services for an effective healthcare delivery model.

Qualifications

• Experience of working within primary care/general practice as a clinical pharmacist.
• Ability to analyse complex technical information and use data to improve clinical outcomes.
• Ability to plan, manage, monitor, advise and review medicine optimisation for long-term conditions.

Responsibilities

• Provide medicines management and prescription support for clinicians (telephone/video/face to face).
• Undertake clinical medication reviews and manage polypharmacy.
• Collaborate with community/hospital pharmacy teams to improve patient outcomes.
• Lead on development of new services or modes of delivery to meet changing healthcare needs.
• Support governance and safety in prescribing practices.
