Job summary

Primary Integrated Community Services in Nottingham is seeking a pharmacist to join the neighbourhood PCN. You will work across two practices initially, delivering medicines optimisation, chronic disease management, and clinical medication reviews as part of a multi-disciplinary team.

You'll provide leadership on medicines safety, support GP staff with prescriptions, and contribute to initiatives improving patient care within the PCN and wider community.

Qualifications

• Master’s degree in Pharmacy and GPhC registration.
• Understanding of GP practices and primary care workflows.
• Experience with medicines optimisation and chronic disease management.

Responsibilities

• Work as part of a multi-disciplinary team in a patient-facing role.
• Provide prescription and medication query support to general practice staff.
• Lead medicines optimisation initiatives and quality improvement.
• Conduct clinical medication reviews and care home domiciliary reviews.
