Job summary

Consett Medical Centre is seeking a pharmacist to join our general practice team. You will conduct clinical medication reviews, support the practice with prescription queries, and work with community and hospital pharmacists to optimise medicines and improve patient safety.

We welcome applicants with a pharmacy degree and GPhC registration; independent prescribing is desirable. The role is patient-facing within a multidisciplinary team in County Durham.

Qualifications

• In-depth therapeutic and clinical knowledge relevant to primary care prescribing.
• Strong communication and interpersonal skills for patient-facing roles.
• Ability to plan, manage and monitor medicines optimisation in core areas.

Responsibilities

• Undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
• Provide primary support to general practice staff on prescription queries and medicines information.
• Reconcile medicines during discharge from hospital or care transfers; ensure continuity of care.
• Collaborate with community and hospital pharmacy to improve safety and effectiveness of prescribing.
• Lead/participate in medicines optimisation initiatives and education for the practice team.
