Job summary

Qualifications
- Experienced Clinical Pharmacist with general practice experience.
- Experience in medicines optimisation and improving prescribing quality and safety.
- Proficient in medication reviews and handling complex regimens.
Responsibilities
- Undertaking medication reviews and Structured Medication Reviews (SMRs).
- Identify and reduce polypharmacy and unnecessary prescribing.
- Reconcile medicines post-discharge with hospital and community teams.
- Address medicines-related problems to reduce avoidable admissions.
- Develop relationships with community and hospital pharmacists and other clinicians.
- Support repeat prescribing and reauthorisation by reviewing requests.
- Respond to medicines-related queries from patients and clinicians.
- Improve prescribing quality through audits, recalls and PDSA cycles.
- Manage practice formularies for safe and cost-conscious prescribing.
- Implement MHRA safety alerts and drug withdrawals.
- Work with clinicians to implement NICE guidance and evidence-based recommendations.
- Attend clinics focusing on medicines optimisation and chronic disease monitoring.
- Review pathology results for patients on prescribed medicines.
