Job summary

Lower Broughton Health Centre in Salford is seeking an enthusiastic Clinical Pharmacist to join our established team. The role focuses on medicines optimisation, prescribing safety and quality improvement to deliver high standards of patient care.

We welcome those with Independent Prescribing qualification or CPPE accreditation. 37.5 hours per week, permanent contract, with NHS pension and annual leave. Salary £49,000–£56,000 depending on experience.

Qualifications

• Registered Clinical Pharmacist with GPhC registration.
• 12+ months experience working in primary care.
• Experience with EMIS
• Independent prescribing qualification (working towards / or completed)

Responsibilities

• Undertaking Structured Medication Reviews (SMRs) to optimise treatment and outcomes.
• Identify and reduce inappropriate polypharmacy and unnecessary prescribing.
• Reconcile medicines after hospital discharge and liaise with patients and hospital teams.
• Address medicines-related problems to reduce avoidable admissions.
• Review repeats and monitor dates; respond to medicines queries.
• Improve prescribing through audits, recalls, and PDSA cycles.
• Support formulary management for safe, cost-conscious prescribing.
• Implement MHRA safety alerts and drug withdrawals.
• Contribute to NICE guidance and multimorbidity reviews.
• Provide medicines information and training to staff and patients.
