Job summary

Primary Care Doncaster Limited is seeking Clinical Pharmacists to join the North Primary Care Network in Doncaster. You will work in a multi-disciplinary team with supervision from a senior pharmacist, across three practices, providing comprehensive pharmacy services and liaison with community pharmacy.

You will optimise medicines, handle prescription queries, improve prescribing safety, educate patients on self-management, and manage repeat prescribing processes.

Qualifications

• Pharmacists educated to degree level and registered with GPhC.
• Possess a Masters Degree (MPharm) or equivalent.
• Ability to work across multiple practices within the PCN.

Responsibilities

• Optimise the use of medicines
• Support the PCNs with regards to prescription and medication queries
• Improve the safety and quality of prescribing
• Providing support and education to patients on self-management and ensure referral to appropriate healthcare professionals if required
• Implement and manage the repeat prescribing process for practice and patient request
