Job summary

Primary Care Doncaster Limited is recruiting Clinical Pharmacists to join the North Primary Care Network (PCN) in Doncaster. You will work in a multi-disciplinary team, with supervision from a senior pharmacist and colleagues including pharmacists and pharmacy technicians, delivering patient-facing pharmaceutical services across three practices.

Candidates must hold GPhC registration, a Master of Pharmacy (MPharm) or equivalent, and may include Independent Prescriber qualifications.

Qualifications

• GPhC registration required.
• Master of Pharmacy (MPharm) degree or equivalent.
• Independent Prescriber qualification or willingness to complete.

Responsibilities

• Take responsibility for chronic disease management within the practice and undertake clinical medication reviews for patients with polypharmacy.
• Provide primary support to general practice staff on prescription and medication queries; support repeat prescription system and medicines reconciliation.
• Collaborate with community and hospital pharmacy to improve patient outcomes and safe prescribing.
