Job summary

Modality Partnership in Lewisham is seeking a skilled Clinical Pharmacist to deliver patient-centred pharmaceutical care within our Primary Care teams.

You will optimise medicines, undertake medicines reviews, and contribute to service development while supporting colleagues across Divisions as we drive innovation and improvements in patient outcomes.

Qualifications

• Qualified Clinical Pharmacist with registration and a GPharm degree.
• Experience in primary care or medicines optimisation is a plus.
• Strong collaboration and communication skills with patients and colleagues.

This is a dynamic, patient-facing Clinical Pharmacist role, focused on delivering evidence-based pharmaceutical care in Primary Care. You will use your expert knowledge of medicines to support patients with specific disease areas and long-term conditions.,

• Clinically assessing, diagnosing, and prescribing (where qualified)

• Contributing to service development and innovative working practices
