Job summary

Modality Partnership’s Jenner Health Centre in Lewisham is seeking a full-time Medicine Care Coordinator to support medicines-related services and help achieve KPIs. You will work closely with the Lead Pharmacist and PCN Manager within a multidisciplinary team to improve patient care and service delivery.

  • Working flexibly and resourcefully to ensure tasks and priorities are completed effectively, In this role, you will work directly with key stakeholders to help harmonise ways of working and improve working practices, contributing to improved patient and staff satisfaction.

As a Medicines Care Coordinator, you will work closely with the Lead Pharmacist and PCN Manager to support the delivery of medicines-related services and achieve key performance indicators and targets outlined within standard operating procedures and the business plan. This is a dynamic role that requires innovation, flexibility, commitment, and the ability to work resourcefully as part of a multidisciplinary team. You will contribute to the delivery of high-quality services while helping to identify opportunities to improve processes, patient care, and the overall patient experience. You will work directly with key stakeholders to help harmonise ways of working, improve working practices, and contribute to sustainable improvements across Primary Care.
