An exciting opportunity for a motivated Clinical Pharmacist to join the NHS team in Wigan. This role involves managing clinical medication, supporting acute conditions, and working within a multi-disciplinary team to enhance patient care. An exciting opportunity for a motivated and enthusiastic ClinicalPharmacist has arisen to join our team. The working hours for this position arefull time, flexible.
This position will support our clinical teams dealing with day-to-dayacute conditions alongside medication management including post dischargesynchronisation, medication reviews and repeat prescribing.
We are looking for an individual who has excellent communication skills andadaptability to the ever-changing NHS. We will provide a friendly andsupportive environment to work in alongside development and training. You will work alongside our existing clinical pharmacists supporting our practices with medicines management.
The post holder will manage and undertake the following duties:
- Clinical medication management and optimisation
- Acute and minor aliment clinics within the candidates competencyframework
- Deal with incoming hospital discharges and third-party letters
- Repeat prescribing
- Supporting with QOF work
- Medicine safety and quality audits
- Care Quality Commission work
- Contractual target-based work
- Deal with medicine related queries
The successful applicant willwork within their clinical competencies as part ofa multi-disciplinary team to provide expertise in clinical medicinesmanagement, provide face to face and telephone structured medication reviews,manage long-term conditions, ensure systems for safe prescribing, addressingpublic health and social care needs of patients at the practice. Thepost-holder will perform review of patients with polypharmacy especially forolder people, people in residential care homes and those with multipleco-morbidities.

This role is pivotal to improving the quality of care and operationalefficiencies so requires motivation to deliver an excellent service withingeneral practice.
Provide proactive leadership on medicines and prescribing systems to themultidisciplinary team, patients and their carers.
Understand the pharmacy role in governance and be able to implement thisappropriately within the workplace
Maximise the use of technology and management systems to providemanagement of safety, risk and information
Focusing on prevention of illness and personalised care, and supportingpatients to make informed decisions about their care and look after their ownhealth, by connecting them with the full range of statutory and voluntaryservices
Deliver training and inform of prescribing updates, mentoring andguidance to other clinicians and staff on medicine issues
Support the practices in improving the health of their patients throughthe rational and safe use of medicines
Undertake responsibility for areas of QoF and Quality Contract andundertake clinical medication reviews to proactively manage patients withcomplex polypharmacy
Co-ordinate and lead meetings ensuring key performance indicators areidentified and plans for achievement shared.
Work with the general practice team to ensure the practice is compliantwith CQC standards where medicines are involved
Provide advice for the GP management of more complex patients /polypharmacy patients
Report medicines related incidents, contribute to investigations androot cause analyses
Participate in serious incident investigations and multidisciplinarycase reviews
Computer data entry/data allocation and collation, processing andrecording information in accordance with practice procedures
Initiating contact with and responding to requests from patients, otherteam members and associated healthcare professionals and providers
Participate in the primary care network MDT liaising with keystake-holders to ensure consistency of patient care and benefit.
