An exciting opportunity for a motivated and enthusiastic Clinical Pharmacist has arisen to join our team. The working hours for this position are full time, flexible.
This position will support our clinical teams dealing with day-to-day acute conditions alongside medication management, including post-discharge synchronization, medication reviews, and repeat prescribing.
We are looking for an individual who has excellent communication skills and adaptability to the ever-changing NHS. We will provide a friendly and supportive environment to work in, alongside development and training. You will work alongside our existing clinical pharmacists supporting our practices with medicines management.
The post holder will manage and undertake the following duties:
The successful applicant will work within their clinical competencies as part of a multidisciplinary team to provide expertise in clinical medicines management, provide face-to-face and telephone structured medication reviews, manage long-term conditions, ensure systems for safe prescribing, and address public health and social care needs of patients at the practice. The post-holder will perform reviews of patients with polypharmacy, especially for older people, people in residential care homes, and those with multiple co-morbidities.

The post-holder will review patients with polypharmacy, especially older adults, residents in care homes, and those with multiple co-morbidities.
This role is pivotal to improving the quality of care and operational efficiencies, requiring motivation to deliver excellent service within general practice.
Provide proactive leadership on medicines and prescribing systems to the multidisciplinary team, patients, and their carers.
Understand the pharmacy role in governance and implement it appropriately within the workplace.
Maximize the use of technology and management systems to provide safety, risk management, and information.
Focus on prevention of illness and personalized care, supporting patients to make informed decisions about their care and to look after their own health by connecting them with statutory and voluntary services.
Deliver training, updates, mentoring, and guidance to clinicians and staff on medicine issues.
Support practices in improving patient health through rational and safe medicine use.
Undertake responsibilities for QoF and Quality Contract areas, and conduct clinical medication reviews to proactively manage patients with complex polypharmacy.
Coordinate and lead meetings, ensuring key performance indicators are identified and shared.
Work with the general practice team to ensure compliance with CQC standards related to medicines.
Provide advice for GPs on managing complex and polypharmacy patients.
Report medicines-related incidents, contribute to investigations, and root cause analyses.
Participate in serious incident investigations and multidisciplinary case reviews.
Handle data entry, processing, and recording in accordance with practice procedures.
Initiate contact and respond to requests from patients, team members, and healthcare professionals.
Participate in the primary care network MDT, liaising with key stakeholders to ensure consistent patient care.
