Job Description:

We are offering a new opportunity for a Hybrid Pharmacist Prescriber. The role includes:

1. Full-time position - 40 hours/week, typically over 4 days
2. Hybrid contract with remote working options
3. Providing primary support to general practice staff on prescription and medication queries
4. Assisting with management of the repeat prescriptions system
5. Conducting medicines reconciliation during transfer of care and ensuring safety protocols are followed, offering expert clinical advice on medications to address public and social care needs within GP practices
6. Providing clinical leadership on medicines optimisation, quality improvement, and managing aspects of the QOF and enhanced services
7. Managing chronic disease areas within the practice and performing medication reviews for patients with complex polypharmacy
8. Facilitating integration between practices, community, and hospital pharmacy services to improve patient outcomes and healthcare access
