Responsibilities and Duties

Clinical Pharmacy in General Practice, our goals areto help keep patients safe and well out of hospital and reduce the demands on generalpractice regards medicine use.The post holder will work within their clinical competencies as part of a multi-disciplinaryteam to provide expertise in clinical medicines management, provide face to face structuredmedication reviews, manage long term conditions, management of medicines on transfer ofcare and systems for safer prescribing, manage repeat prescription authorisationsprocesses, while addressing both the public health and social care needs of patients in theGP practice. The post holder will provide leadership on quality improvement and clinical audit and wellas managing some aspects of the Quality and Outcomes Framework. This role is pivotal toimproving the quality of care and operational efficiencies so requires motivation andpassion to deliver an excellent service within general practice.As well, the post holder will develop and encourage a positive culture of medicinesoptimisation. General requirementsThe post holder is expected to Support a workplace culture that values equality and diversity principles towardsother staff and patients. Ensure principles of openness, transparency and candor are observed and upheld inall working practices including respect for information and clinical governance, andsafeguarding responsibilities.
Personal & Professional DevelopmentThe post-holder will take responsibility for their own development, learning andperformance byactively participating in any external training GP practicesPromote high quality cost-effective and evidence-based use of medicines within thePCN, Work within the Pharmacy Team, with other clinical pharmacist and Pharmacy Technician, to respond tomedication-related queries generated by patients, GPs, other health careprofessionals and staff. Undertake clinical medication reviews either face to face or via telephone supporton identified stratified-risk patients that may include residents in care homes, intheir own, with long-term conditions, and/or learning disabilities. Identify and produce recommendations for medication optimisation for GPs and other multi-disciplinary team members. Contribute to the safety of drug monitoring processes and drug alerts. Work with your line manager to undertake continual personal and professionaldevelopment, taking an active part in reviewing and developing the role andresponsibilities. Adhere to organisational policies and procedures, including confidentiality,safeguarding, lone working, information governance, and health and safety. Duties will vary according to the demands on the Practice.
